The Supported Living Manager-2 (SLM-2) holds a crucial role in managing two site locations, overseeing scheduling, medical appointments, and banking needs for persons supported. Responsibilities include entering PCISP goals into Provide, completing Quarterly summaries, and ensuring compliance with the DDRS Portal. Acting as an advocate in mandatory meetings, the SLM-2 provides supervision and support to Direct Support Professionals (DSPs) and mentors individuals with disabilities.

 

The SLM-2 actively ensures person-centered thinking in all services and the daily execution of ISP goals. This includes overseeing medication administration, documentation, and coordination of appointments. Supervising DSPs, conducting training, and arranging staff meetings are integral to the role. The SLM-2 monitors home maintenance, oversees repairs, and collaborates with the Assistant Director of Waiver Supports for budgeting and revenue maximization.

 

You’ll coordinate with DSPs to follow each person's ISP, facilitate weekday activities, and address concerns promptly. Active participation in team meetings and training, maintaining Medicaid Waiver Supported Living files, and promoting natural support systems are key responsibilities. The SLM-2 maintains regular communication with persons served, employees, and families.

 

This role actively promotes resource efficiency, cooperates with auditors, and addresses internal and external audit findings promptly. A broad understanding of the organization's program areas is maintained, and all restrictions are communicated and implemented with HRC approval. The SLM-2 actively engages in professional development opportunities and is a member of the Extended Leadership team.

Requirements

 Preferred Minimum Qualifications: 

  • Minimum of a high school diploma/GED.
  • Prefer 2 years experience in the field of Human Services.
  • At least 18 years of age.
  • Basic language, written, and computer skills necessary for communication and documentation.
  • Demonstrated ability to support people with disabilities.
  • Ability to work a flexible schedule if needed.
  • Valid driver’s license, insured vehicle, and safe driving record.
  • Ability to speak, hear, and read the English language.
  • Essential physical skills: be able to run, squat, stoop/bend, kneel, climb stairs, lift up to 50 pounds, push and pull against potential resistance, and implement non-physical and approved physical interventions and CPR with people supported when needed.
